Updated information on receiving the COVID-19 Vaccine courtesy of State Senator Alessandra Biaggi & County Executive George Latimer:: Today Governor Andrew M. Cuomo announced that New Yorkers within Phase 1B can now begin scheduling appointments with individual providers, including pharmacies, local health departments, and hospitals, to receive the COVID-19 vaccine. New Yorkers who fall into Phase 1A of the distribution plan will continue to be eligible for the vaccination. Those now eligible under Phase 1B to receive the vaccine include: Individuals 75 and olderFirst respondersPublic safety and corrections officersTeachers and other school staff; in-person college instructors Childcare workersPublic-facing grocery store workers Transit workersIndividuals living and working in homeless shelters Scheduled appointments are required to receive a vaccination. To determine your eligibility, access a list of nearby providers, and schedule your vaccination appointment, use New York State's new 'Am I Eligible" app. New Yorkers can also begin calling the New York State Vaccination Hotline at 1-833-NYS-4VAX (1-833-697-4829). Prior to receiving your scheduled vaccination, you must complete the New York State COVID-19 Vaccine Form. This form can be completed online at forms.ny.gov/s3/vaccine, and you will receive a submission ID. New York State has established a network of distribution sites that will supplement the work being done in hospitals to prevent any one hospital from becoming overburdened. This new network will utilize doctors' offices, Federally-Qualified Health Centers, county health departments, ambulatory centers, and pharmacies to get doses in the arms of eligible New Yorkers. Pharmacies will be providing vaccines for New Yorkers aged 75 and older, while hospitals will continue to prioritize vaccinating healthcare workers in Phase 1A and local health departments and union-organized efforts will serve essential workers in Phase 1B. Additionally, the Department of Health is setting up 20 mass distribution sites throughout the state over the next several weeks to support all categories of eligibility.
